Our consultative nurses promote the health and well-being of disabled individuals through coordination of care, monitoring their health care needs, linking individuals to community resources and assessing their individualized needs. Our nurses are specially linked to each individual based on their needs and our nursing area of expertise.

Individuals enjoy the comfort of a clean and spacious home that helps them learn independence in both social and adaptive skills with the assistance of trained, highly qualified staff. Our individuals receive the assistance that they need while also integrating into their community.

Under our community transition program, Abled Directions helps individuals transition from an institutional setting to a community setting. We also help individuals promote their independence by transitioning to their own private residence in the community.
